WAIVER, QUITCLAIM AND RELEASE OF LIABILITY

In consideration of participating in Passion Sports Program (Basketball, Volleyball & Badminton), such as athletic sports camp, related-tournament and other sports activities, the undersigned acknowledges, appreciates and agrees that:

I/We knowingly and freely assume all such risks both known and unknown and have to assume full responsibility for player’s participation.

I/We guarantee that my child is physically fit to undergo different sports activities such as tournaments, tune-up games, camp drills and recognizes the physical aspects associated with Passion Sports activities.

I/ We willingly agreed to comply with the stated and customary terms and conditions for my participation and that the organizers own the images and videos of participants taken from this event and are authorized to use in any publicity and promotional relating to the event.

I/ We willingly agreed that Passion Sports and its venues will not be held liable of any damages/loss to personal property.

In consideration for participation and acceptance for the entry of my child to this event, I shall agree that as parent/guardian do hereby waive and forever release all rights to claim or damage by on or behalf the registrant’s actions against Passion Sports, Coaches, Venues and other persons handling the related event. Registration fee is non- refundable and non-transferrable.
